These Terms and Conditions govern your use of HealthyActiveLives.co.uk (the "Website"). The Website is operated by DDM Health Ltd (company number 07975193) on behalf of Solihull Metropolitan Borough Council for the Healthy Active Lives service ("Healthy Active Lives", "we", "us" or "our").

By accessing or using the Website, you agree to these Terms and Conditions. If you do not agree, you must not use the Website.

1. About the Website

The Website provides:

  • general health and wellbeing information, resources and signposting;
  • details of local support, programmes, activities and targeted services;
  • digital tools such as forms, calculators, eligibility checks or similar features; and
  • ways to ask for contact, make an enquiry, or submit a referral or enrolment request.

The Website is intended for information, signposting and service access support. It is not a replacement for professional healthcare advice or emergency support.

2. Medical disclaimer and emergencies

Content on the Website is provided for general information only. It is not medical advice, diagnosis or treatment, and it must not be relied on as a substitute for advice from a GP, pharmacist, nurse or other qualified healthcare professional.

If you have concerns about your health or wellbeing, you should seek advice from an appropriate professional.

  • decline, defer or close a referral or enquiry;
  • redirect you to another provider or service where that is more appropriate; or
  • change or withdraw a service, programme, eligibility rule or Website feature at any time.
